Job Description: This position is responsible for assisting patients referred from Psychiatry, Emergency Department, and other units located within the facility in achieving their discharge goals for follow-up treatment. The Behavioral Health Patient Navigator is instrumental in educating patients on how to access available mental health resources and services within their community.

We are seeking a highly organized and detail-oriented individual to join our team. The successful candidate will be responsible for ensuring seamless patient intake, referral, and follow-up processes while maintaining 100% HIPAA compliance.
Key Responsibilities:
- Maintain availability via work phone 24/7, especially during working hours.
- Coordinate patient intakes and referrals to therapists, medication management, or both.
- Track therapy and medication management visits, check-ins, and outcomes.
- Connect patients with necessary services, ensuring attention to detail and meeting all patient needs.
- Collaborate with the Ambulatory Organizer on iCentra for medication management visits (9 doctors).
- Complete and fax vouchers for confirmed visits to respective doctor's offices.
- Monitor pharmacy reports for grant patients receiving grant services.
- Compile monthly medication management reports and initiate next month's report.
- Generate quarterly pharmacy reports and submit to AUCH for pharmacy reimbursement.
- Excellent organizational and communication skills.
- Ability to maintain confidentiality and adhere to HIPAA guidelines.
- Strong attention to detail and problem-solving skills.
- Proficiency in electronic health records (EHRs) and other relevant software.
Job Essentials:
- Meets with patients in need of behavioral health services who have been referred from inpatient units, Emergency Departments, and Access Centers to review their discharge plan.
- Assists patients in connecting with referred resources.
- Initiates timely follow-up calls with the patient after their discharge to ensure they are following their discharge plan and receiving maximum benefit from available resources.
- Gathers needed data and completes the Referral Tracking Sheets and other required logs timely and accurately.
- Coordinates patient care needs with care providers and other hospital resources, including attending meetings where patient needs may be addressed or need to be represented.
- Establishes and maintains an accurate resource list of treatment facilities and community resources, updating the list as needed.
Minimum Qualifications:
- Excellent interpersonal and communication skills.
- Experience adapting quickly when the need arises.
- Knowledge of available behavioral health community resources.
Preferred Qualifications:
- Three years of experience working with Behavioral Health Patients in a behavioral health care setting.
- Post high school education in a related field of study.
- Experience with discharge planning for behavioral health patients.
